Risk Assessments
BoolCare's AI risk assessment tool guides you through a structured question flow tailored to the type of risk being assessed, then produces a detailed, personalised risk assessment document.
How it works
Unlike many systems that provide one-size-fits-all risk forms, BoolCare lets you define the risk type yourself (e.g. Falls, Skin Integrity, Choking, Moving & Handling). The AI then asks the right questions for that risk type and writes a full assessment based on your answers.
Creating a risk assessment
- Open a resident's profile.
- Scroll to the Risk Assessments section and click New risk assessment.
- Enter the risk type — a short description such as Falls, Pressure Sores, or Wandering.
- Answer the wizard questions. These adapt to the risk type you entered.
- Click Generate assessment.

Set a review date when creating the assessment. The AI may suggest a review frequency (e.g. monthly for high-risk, quarterly for low-risk) based on the severity of the risk identified.
The dashboard flags assessments that are due or overdue.
Viewing all risk assessments for a resident
From the resident profile, the Risk Assessments section lists all assessments with their type, date, kanban status, and next review date.
